Project Upstream provides a range of
support services to persons with mental illness. Services
include:
- Intensive Mental Health Case
Management - Our operational core
- Family-focused case management
(liaison with network and coordination of services);
- Advocacy;
- Informal counselling;
- Life skills teaching/coaching (ex.
budgeting, nutrition, cooking, cleaning, medication
management);
- Group facilitation;
- Crisis prevention/intervention;
- Goal development and
strategies/support for achievement.
- Practical supports and assistance
Project Upstream
also offers housing in:
- Shared homes;
- Shared apartments; or
- Single apartments.
Project Upstream
housing offers:
- Fully furnished rent
geared-to-income units
To apply for case management services
from Project Upstream and/or other mental health service
providers in Ottawa-Carleton, an application form must be
submitted to the Mental Health Community Support Services (MHCSS)
via the Canadian Mental Health Association. For more
