Case Management and Affordable, Permanent Housing

Project Upstream provides a choice of flexible support services and/or housing options to individuals with long-term mental illness. We pioneered a family case management approach and continue to serve clients both individually and within a wide range of family situations.

Our professional case managers offer diverse services, including:

• Personal counselling
• Help with establishing meaningful goals
• Life skills coaching
• Support in achieving goals
• Advocacy
• Referral to other relevant services
• Assistance with medication management
• Budgeting
• Assessment
• Service coordination
• Emotional support and encouragement

We provide decent, affordable and supported housing (one or two bedroom apartments for men or women and group homes for men). All housing units are furnished.
